Hard to tell, but in my experience the scoby "is born" as an even film on the liquid's surface rather than separated particles.
4 weeks is a long time, I don't think it's working. But or course I can be wrong
It doesn't look like it. It starts as a white film and gets thicker over time. After 4 weeks, you should have a thick layer over the entire surface of the liquid.
